The first identifier is mandatory and is the unique national identifier for this instance of a planned medication administration. The second identifier carries a local identifier allocated by the source system.

The Identifier Global datatype flavour is used to carry the first identifier as follows:

  • The root attribute shall contain a DCE UUID

The Identifier External datatype flavour is used to carry the second identifier as follows:

  • The root carries an OID for the local identifier scheme
  • The extension carries a human-readable identifier from that scheme

One or more SnomedCT code identifying the act as describing planning the administration of medication. 

The Concept Descriptor (CD) datatype shall be used to carry this information as follows:

  • The display text shall carry a description associated with the code in SnomedCT

The mandatory status of the planned medication administration, e.g. active, suspended, completed.

Required if status is "aborted" or "cancelled"  

 The Coded Only datatype flavour is used to carry this information as follows:

  • The code attribute will contain one of the following values from the list of codes in the Act Status vocabulary
  • cancelled  - the (author) has cancelled the plan
  • active - the plan is active but hasn't been accepted yet by a performer
  • completed - the plan has been accepted by a performer.
  • aborted - the plan has been aborted.
[0..1] effectiveTime (GTS)

This will be constrained to IVL < TS > in this release.

The start and end dates of the planned  medication administration. The start end is when it is / was planned to start administering medication and the end date is when it is / was planned to stop administering medication.

A date range shall be carried as follows:

If both a start date and an end date are present the Date or Time Interval Complete datatype shall be used:

  • The low attribute shall contain the start date
  • The high attribute shall contain the end date.

If only a start date is present the Date or Time Interval After datatype shall be used:

  • The low attribute shall contain the start date.

If only a start date is given then a review date of the planned medication is required.

The date and time that the planned administration was recorded on the source system.

[0..1] doseQuantity (IVL<PQ>)

The quantity of substance used for each administration when it is measured as a dosage, e.g. 2 tablets, 10 ml. Either this attribute or the following one should be used.

"O" (Overriding) - the association replaces any context of the same or more specific type that has been conducted from the parent act.

"P" (Propagating) - the association is conducted to all child acts where the ActRelationship.contextConductionInd is set to "true"

The time which or during which the performer attempted to inform the patient.

[0..1] modeCode (CE {CNE:ParticipationMode} )

The mode in which the attempt to inform the patient of discontinuation of supply of medication took place.

The Identifier Global datatype flavour is used to carry the first identifier as follows:

  • The root attribute shall contain a DCE UUID

A SnomedCT code identifying the reason for discontinuation of the medication supply. 

The Concept Descriptor (CD) datatype shall be used to carry this information as follows:

  • The code attribute shall contain a code from the relevant vocabulary
  • The code System attribute shall contain the OID for SnomedCT
  • The display text shall carry a description associated with the code in SnomedCT
  • the original text attribute may be used if required
  • qualifier(s) may be used if required.

An NHS number is carried in the following way:

  • The root attribute will contain an OID with the value "2.16.840.1.113883.2.1.4.1"
  • The extension attribute will contain the NHS number.

Only new permanent NHS numbers can be used to identify patients.
