CABAnnotation codes for acts in Choose and Book query reponses

issue Description:

Currently, there are place-holders in the query response models for SNOMED codes to hold meaningful codes for the following acts:

OutlineReferralList, Service, AdviceAndGuidanceRequest, UninitialisedRequest, ActiveAppointment

The business team have requested that SNOMED codes not be used, as there are no appropriate codes and no processing witll be done on this field to necessiate their addition to the SNOMED terms list.

Proposed solution

A new vocabulary was created for MIM 5.2 called CABAnnotation, and this will be added to in order to provide meaningful fixed codes for the acts in question.

No appropriate codes exist in SNOMED for these acts, so if this change is not made, there will be no way to populate the code field - the code field should identify the act, and if it doesn't, the model is not HL7 compliant.
